Secretagogin在胃肠道类癌诊断中的作用

【摘要】  目的 探讨Secretagogin在胃肠道类癌诊断中的作用。方法 采用免疫组织化学技术检测27例胃肠道类癌(6例非典型类癌)中Secretagogin、嗜铬素A(CgA)、突触素(Syn)、神经元特异性烯醇化酶(NSE)的表达，分析其表达差异性;并比较它们在典型类癌与非典型类癌中表达情况。 结果 胃肠道类癌组织中Secretagogin、Syn、CgA、NSE的阳性表达率分别为96.3%、100.0%、40.7%、81.5%，四者之间差异有统计学意义(χ2=23.7，P<0.01)。CgA在非典型类癌中的表达高于典型类癌(Z=-2.098，P<0.05)，其余三者均无差别。结论 在绝大多数胃肠道类癌组织中Secretagogin呈阳性表达，且表达程度高于CgA; Secretagogin可以作为一个新的神经内分泌标记物用于临床胃肠道类癌的诊断。

[ABSTRACT] Objective To assess the value of Secretagogin in the diagnosis of gastrointestinal carcinoid. Methods The expressions of Secretagogin, synaptophysin (Syn), chromogranin A (CgA) and neuronspecific enolase (NSE) in 27 cases (six were untypical) of gastrointestinal carcinoid (GC) were detected immunohistochemically. Results The positive expression rate of Secretagogin, Syn, CgA and NSE in carcinoid was 96.3%, 100.0%, 40.7% and 81.5%, respectively. The differences among the four items were significant (χ2=23.7,P<0.01). CgA expression in untypical carcinoid was higher than that in typical one(Z=-2.098,P<0.05), for the rest three markers, there was no significant difference among them. Conclusion Secretagogin positively expresses in the most majority of carcinoid, and its expression is stronger than that of CgA. Secretagogin can be used as a novel neuroendocrinal marker in the diagnosis of carcinoid of gastrointestinal tract.

　　类癌属于神经内分泌肿瘤，比较少见，占全部恶性肿瘤的0.05%～0.20%，好发于消化道，占胃肠道肿瘤的0.4%～1.8%。Secretagogin是最近研究较热的神经内分泌标记物，最早发现于胰岛细胞，为钙离子结合蛋白，属于钙结合蛋白中的感受器家族，定位于染色体6p22.3?p22.1，在神经内分泌肿瘤组织中高表达，其与传统的神经内分泌标记物嗜铬素A(CgA)、突触素(Syn)以及神经元特异性烯醇化酶(NSE)相同，均定位于胞浆中，是一种新型神经内分泌标记物[1]。对Secretagogin深入研究可能为类癌的诊断提供新的依据。

　　1 材料和方法

　　1.1 材料

　　1.1.1 标本 收集青岛大学医学院附属医院、青岛市市立医院、青岛市中心医院及中国人民解放军第401医院2006年5月—2009 年12 月保存的消化道类癌标本27例，病人中位年龄60岁。所有标本均经40 g/L中性甲醛固定，常规石蜡包埋，4 μm厚连续切片。所有类癌组织均经资深病理医师及免疫组化检查证实。

　　1.1.2 试剂 兔抗人Secretagogin多克隆抗体由浙江大学来茂德教授馈赠;山羊抗兔二抗(即用型)，CgA、Syn、NSE即用型一抗，PV?6000试剂盒，DAB显色试剂盒均购自北京中杉金桥生物技术有限公司。

　　1.2 方法

　　1.2.1 检测方法 Secretagogin免疫组织化学染色采用EnviSion法,切片在pH 6.0的柠檬酸盐缓冲液中微波抗原修复，一抗1∶400稀释。CgA、Syn及NSE免疫组织化学染色采用PV?6000法，具体步骤严格按照试剂盒说明操作，DAB显色试剂盒显色。阳性对照用已知阳性片(Secretagogin胰岛组织);阴性对照省略一抗，以PBS 代替。
